The Clinical Coordinator Assessment
If it seems like a good match, our Clinical Coordinator will call you.
This conversation goes deeper. They’ll ask about your parent’s medical needs and day-to-day life. They’ll explain how our physician-led program works and what makes it different from traditional home care. One of our physicians oversees every care plan from the start.
Ask specific questions about the process. If something requires medical expertise, the Clinical Coordinator will consult with one of our physicians.
If we’re not the right fit, we’ll point you toward other resources.

We schedule a meeting with you, your parent, and any other family members who should be involved. The physician and Clinical Coordinator walk through the assessment findings and recommend next steps. You’ll discuss what matters most right now, what to address first, and what can wait.
Medical information is hard to retain, especially when you’re worried about someone you love. We take the time needed to make sure everyone understands the plan.
Ongoing Physician-Led Care
Our Clinical Coordinator becomes your regular point of contact after the initial assessment. They check in proactively – weekly at first, then as needed – to see how things are going and catch issues early.
They coordinate all follow-up appointments with the physician, physiotherapist, or other team members. Every six months, the physician and Clinical Coordinator conduct a thorough review at your parent’s home, similar to an annual physical but tailored to their current needs.
